Eligible candidates may appear for interview on 10 December 2019 along with duly filled application form in the prescribed format (attached below) and self-attested copies Certificate in support of age (10th Certificate), qualification, experience, Medical registration certificates of DMC, mark sheets of MBBS Part, II & Final year, undergraduate / Post Graduate, Cast certificates and original documents for verification.
