- Date of Birth Proof
- Self attested copies of mark sheets of all MBBS Examination
- No Objection Certificate issued from college/University.
- Indian graduates are required to submit the NOC issued from their parent college/university in favor of this institute along with an undertaking that colleges/ university is duly recognized by MCI before applying internship.
- Screening Test Passing Certificate issued from NBE is mandatory for foreign candidates
- Copy of Aadhar Card & one local address proof
- Two Passport size photograph
